What is Alternative Family Living?
In North Carolina, AFL is a residential support model under the NC Innovations Waiver, where individuals with intellectual and developmental disabilities (ID/DD) live with a family (licensed or unlicensed) that’s trained to provide personalized care. It’s designed to be person-centered, meaning the individual’s preferences, goals, and strengths guide the support they receive.
What Makes AFL Unique

Family-Based Environment: Instead of institutional care, individuals live in a home setting with caregivers who treat them like family.

Individualized Support: Services are tailored to the person’s needs — from daily living skills to emotional support.

Flexibility: AFL homes can be licensed or unlicensed, depending on the provider and the individual’s needs.
